As I approach 40 years in practice, I would like to inform you that I will be retiring from general practice on 17 June 2026. My last in-office appointment will be on 13 May 2026.

It has been an honour to serve you as your family physician. I am deeply grateful for your trust and wish you continued health and well-being.

NEW ALERT – INTRODUCING OUR NEW PHYSICIAN

I am very pleased to announce that we have secured a highly qualified replacement physician, ensuring continuity of care for our patients.

We are delighted to introduce Dr. Maryam Yavari, MD, PhD, ND, CCFP, who will officially join the Wrellen team on June 1, 2026. As part of a planned transition, Dr. Yavari will be taking over the care of my patient roster.

Dr. Yavari brings over 15 years of experience caring for individuals and families in both community and hospital settings with a strong foundation in evidence-based, holistic family medicine. Beyond her clinical work, Dr. Yavari has extensive experience in medical education and research. She has taught at the University of Toronto and internationally as a lecturer and assistant professor, supervised medical students, residents, and graduate students, and contributed to over 50 peer-reviewed publications and several books. She has also held leadership roles in primary care research and served on multiple international boards promoting healthcare innovation and education.

With a warm and attentive approach, Dr. Yavari looks forward to building long-term relationships and providing trusted, continuous care for you and your family.

Patients who choose to remain with Dr. Yavari will not need to have their charts transferred via Docnote.

Further details regarding the process for transitioning medical records to her care will be communicated shortly.

MEDICAL RECORDS AND CHART TRANSFERS

In accordance with the requirements of the College of Physicians and Surgeons of Ontario (CPSO), all medical records will be securely stored for the required retention period following my retirement.

Patients may request copies of their medical records or transfers to a new health-care provider at any time.

To support a secure and efficient process, administrative and medical record requests will be managed through Docnote, a secure online platform.

Once registered, you will be able to submit and track your requests directly through the platform.

REQUESTS REQUIRED TO BE SUBMITTED VIA DOCNOTE

The following requests must be submitted through Docnote:

• Sick notes, Attending Physician Statements, and other medical forms
• Insurance, disability, and work-related documentation
• Prescription renewals
• Travel immunizations
• Immunization and medical procedure reports

For patients choosing an alternate physician.

Patients who choose not to remain with the practice are encouraged to begin searching for a new family physician as early as possible. The following resources may be helpful:
