As I approach 40 years in practice, I would like to inform you that I will retire from general practice on 17 June 2026. My last in-office appointment will be on 13 May 2026.

It has been an honour to serve you as your family physician. I deeply appreciate your trust and wish you ongoing health and well-being.

Medical Records and Chart Transfers

As required by the CPSO, your medical records will be securely stored for the required period after my retirement. You can request copies or transfers to new health-care providers at any time.

To help manage these requests efficiently, I have registered with Docnote, a secure online platform. Patients are required to use Docnote to submit and track requests such as chart transfers and other administrative tasks. This process helps ensure the timely and secure handling of your information.

To support an organized and timely process, the following requests will be required to be submitted through Docnote:

• Sick notes, Attending Physician Statements, and other medical forms
• Completion of insurance, disability, or work-related documentation
• Prescription renewals
• Travel immunizations
• Immunization & Medical procedure reports
